by coincidence は「偶然に」
by のイメージは「近接・そば」→ そこから「手段・方法」を表す使い方が派生している
「ゴール」を達成する「手段・方法」は「そば」にあるため
このフレーズは「偶然という手段によって＝偶然に」となる

感情の原因を表す節
was surprised の後ろに、節＝you knew who he was が置かれ、説明関係が生じている
「ビックリした」の説明なので、その「原因」となる

作り方はやっぱり単純。the time まで言い切ったら、「そのとき何が起 こったのか」をwhen 以下で説明します。
